How often do Tortoise's need to be wormed?

Depends how they are kept and what their origin is. RCVS recommend annual worming mid season. Given a huge are for roaming worming is less necessary than when kept in small areas, where re infection is higher. Imports should always be wormed on entry to UK and then annually ;)

So is one dose per year enough or do you have to do three doses spaced over a few weeks?

If you see worms after the first dose then dose again in a week. It's easy to see little worms in the water dish or bath, bigger ones are more obvious. Don't do it near to hibernation but when they are most active and eating well.
